Recent investigations by the Parliamentary and Health Service Ombudsman (PHSO) have unveiled critical failures in the interpretation and management of medical imaging within the NHS, leading to severe patient harm, including avoidable deaths and delayed cancer diagnoses.
The severity of these failures is starkly illustrated through several distressing cases highlighted by the BBC.
An 82-year-old man from Buckinghamshire sought emergency care at Wexham Park Hospital five times between August and October 2021. Despite a scan in August revealing a bowel lesion, clinicians reported no abnormalities, resulting in a six-week delay before surgery was performed. An October X-ray indicating a small bowel obstruction went unreviewed, and the patient was discharged without appropriate intervention. By the time cancer was diagnosed later that month, the disease had progressed significantly.
The patient’s prolonged suffering culminated in his tragic decision to end his life, leaving a note expressing his inability to endure the pain any longer. The PHSO concluded that inadequate pain management and diagnostic failures likely contributed to this outcome.
In another harrowing case, a 54-year-old man who fell ill abroad had a brain tumour misidentified as benign by NHS specialists, despite scans suggesting malignancy. This misdiagnosis led to critical delays in necessary treatments such as chemotherapy and radiotherapy. Tragically, he passed away just one month after surgery confirmed the tumour’s aggressive nature. The PHSO indicated that an earlier correct diagnosis could have extended his life by several months.
These cases are not isolated incidents. The PHSO’s 2021 report highlighted ongoing issues in the reporting and follow-up of X-rays and scans across NHS services, emphasising the need for system-wide improvements to ensure timely and effective management of medical imaging.
